If I remember correctly (its been a 6 months since I was last in the same building with the machines that had Ultras in them.) Its similar to NE2000 style NICs and you specify the IO and IRQ on the calling line for module when it loads or as an append eth0="io=0x???" in lilo if its a kernel driver. Now I haven't used Debian in a long time so I'm not sure where you would spec it for the module.
