I do a OOWriter template with fields to fill in.
I keep records of items I sell in an OOCalc spreadsheet

I don't have tons of customers (the customers I have are GREAT!) but I 
feel it should be simple until something complex is needed. Then I'll 
write it in PHP make it open and not worry about paying for it.
