I've been using a small VoIP shop to provide call termination/origination for work: http://www.sentinelcommunications.com/ Yes, they have a crappy website, but their VoIP service and support have been great. They use asterisk in their datacenter, and are very asterisk friendly. When initially implementing my asterisk server, I ran into a few obscure issues - they asked me to send along a verbose SIP log, which I did. Within minutes, they had found the source of the problem and sent me the appropriate asterisk configs to resolve the issue. If you do end up using these guys, tell John I sent you. -Erik
